I have pencil copies of page 4 of the FTB 3500 with 2010 through 2014 in actual and 2015 as the current year.  I'll update 2015 and 216 to actual and do a 2017 projection within the next couple of days.  that same information on printed copies of form 199 but since only the current year form can be submitted on-line I didn't do anything with them.  Also,, I do not know the answer to one of the questions, do we have a IRS form 023/1024 pending.  My guess is no.
I do not have any non-financial information, but do have a blank copy of the 2014 version of the form (except for section G for some reason).
I am available pretty much anytime through July 24th but am leaving for Florida for a month on the 30th.

Per instructions from the ACCRS Board meeting of June 9,2017, I looked into the issues of the "Notice of Suspended Status"dated 04/26/2017.  Though the notice camefrom the Franchise Tax Board (FTB), the office of the California Secretary ofState is also involved---the offices are independent but interact with eachother.   Office of theSecretary of State (SOS) Our last filing with the Secretary of State was on July 15,2015.  A copy of that filing and theprior filing are attached.  According tothat office, we are current through 2017. About 90 days prior to a renewal deadline, a notice will be mailed to myaddress---so, about September/October.   SOS phone No.: 916-657-5448ACCRS corporate ID: C0387677   Franchise TaxBoard (FTB) As it turns out the term "Exempt" can mean anumber of things.  In the case of ACCRS vs. FTB,the term means exempt from paying an $800 fee to the FTB each year.  Per the notice, we have 90 days to file Form 3500along with a $25 filing fee---July 24, 2017 if I counted right.  The person I talked to instructed us to fillout the first eight pages plus Section G (Social and Recreational Organization)of that form.  Along with that form, weneed to include a copy of the Articles of Incorporation (a pdf copy is attached) and a set of Bylaws also attached.  That filing will start the ball rolling andfurther instructions will follow.  I wasfurther told those instructions would include filing Form-199 or Form-199N forthe years of 2010-2016.  The version ofForm-199 is determined by some threshold of income or assets.     Form-3500 and its instruction book are also attached.  Also, Form-199 for 2016 and its instructionbook are attached.  To find FTB forms, goto: www.ftb.ca.gov/forms/search   FTB phone No.: 916-845-7161

Dennis, Dean, and I started filling out Form-3500 two years ago, but didn't complete it.  Perhaps Dennis has that information.  I recommend that Dennis and Dean fill in Form-3500 as much as they can, and then have a meeting of Marc, Dennis, and Dean to complete the form.  Since Gary has a treasure trove of knowledge of the club, his input would be helpful.  If you wish, I am available to assist.  Since time is of the essence, perhaps the meeting could occur at the club during Fair time on an evening when the Fair is closed.  This is up to Marc.
